STYLE YOUR LIFE CONTRACT

Dr. Shelley Nash Coaching (hereinafter referred to as “Dr. Shelley Nash”, “coach” or “company”) agrees to provide the program, STYLE YOUR LIFE (hereinafter referred to as “program” to you, (hereinafter referred to as “you”, “I” or “client”).

1. As a client, I understand and agree that I am fully responsible for my physical, mental, and emotional well-being during my coaching conversations, including my choices and decisions.
2. I understand that coaching is a relationship with my coach that is designed to facilitate the creation of business and personal goals.
3. I will not use coaching as a substitute for counseling, psychotherapy, mental health care, or substance abuse treatment.
4. I give my coach permission to keep a confidential record of my name and contact information.
5. I understand that all material, session recordings, interaction between coach, client, and group remain sole property of Dr. Shelley Nash and can be released, used for marketing purposes, and reproduced at her discretion. 
6. I agree that I am solely responsible for my own actions, behaviors, and results.
7. I will be accountable for the completion of any actions, planning steps, and activities generated from coaching and group sessions.
8. I, the client, understand I am solely responsible for all the outcomes I achieve during this coaching program. Dr. Shelley Nash is not responsible for my outcomes or results. 
9. I am requesting and committing to the services of Dr. Shelley Nash’s coaching in the form of the STYLE YOUR LIFE program for the entire 4-month term.

Facebook Group & Client Membership Site

Program clients will have access to a private Facebook Group and Client Membership Site. All program clients will have access to the current STYLE YOUR LIFE material. You will have life-time access to the STYLE YOUR LIFE materials in the membership site. Clients will maintain access to the Facebook Group and their membership site while their account is kept current. Once you have completed the program you will no longer have access to the Facebook group and weekly group coaching. If you withdraw from the program, or are removed from the program, you will NO LONGER have access to the program. 

All Deposits and Payments are NON-REFUNDABLE during the 4-month term. If you choose to leave before you have completed your 4-month term, there will be no refund. 

If you are making monthly payments over your 4-month term, you will make your payments on the due date each month. If you choose to leave before you have completed your 4-month term, your FULL unpaid program balance is due immediately.  

The CLIENT checks the checkbox at the time of purchase, in acknowledgment and agreement of this contract, and as indication of their full compliance with the above terms and conditions and understanding of the services provided.  [I offer the checkbox as my signature in full agreement and compliance with the terms and conditions of this contract.]
